Our metal bracelet displays the internationally recognized Staff of Aesculapius medical symbol on the front along with the words Wisconsin Do Not Resuscitate EMS. The patients first & last name must be engraved on the first line of the back of the bracelet, however there is room on the back to add other health concerns, if desired.
A copy of your completed Wisconsin doctors form F44763 has to be mailed to StickyJ Medical ID prior to releasing your order.
