StickyJ Medical ID Jewelry is the authorized vendor for South Carolina Do Not Resuscitate (DNR) bracelets. The patient’s first and last name must be engraved on the first line of the back of the bracelet, however there is room on the back to add emergency contacts or other health concerns, if desired.
A copy of your completed South Carolina Do Not Resuscitate form must be mailed to StickyJ Medical ID prior to releasing your order.
