About StickyJ Medical ID DNR and POLST Bracelets
Georgia residents with a Do Not Resuscitate or POLST order signed by their attending physician may wear an authorized bracelet from StickyJ Medical ID to indicate that a DNR or POLST order is in effect.
- For DNR, the back of the bracelet must specify the patient's name, the name of the physician who issued the DNR order, the physician's telephone number, and the date of the DNR order. This isn't required for POLST.
- To issue a Georgia DNR or POLST bracelet, we must first receive a copy of your physician order signed by your attending physician. You can either scan the document and upload it at the time you place your order, email it to CustomerService@stickyj.com, or fax it to 727-499-0989.
Georgia requires that you have a copy of your POLST order with you at all times when you are out in the community. StickyJ Medical ID provides specialized identifier bracelets designed to help Emergency Medical Services (EMS) personnel in Goergia locate your POLST order quickly. The bracelets include instructions for EMS to check your purse or wallet for your medical order.
QR Code Option for Quick DNR or POLST Access
For a modest additional fee, StickyJ Medical ID offers an optional QR Code feature with your medical ID bracelet. If you select this option, you can send us a copy of your DNR or POLST order, which we will securely digitize and upload to our protected online database. The QR Code, engraved on the back of your bracelet, allows paramedics and emergency responders to use a smartphone to scan and immediately view an image of your DNR or POLST order. This can be extremely useful when time is of the essence.
To add the QR Code option, you must either scan-and-upload OR mail a copy of your Do Not Resuscitate or POLST order to StickyJ Medical ID. Once received, we will securely store your document online, making it accessible to EMS personnel whenever necessary. (Note: if you are planning to scan and upload your DNR, we recommend using Adobe Scan. It's a free app available for both Apple and Android phones).
Features of the Georgia Bracelets
The Goergia Do Not Resuscitate and POLST ID bracelets by StickyJ Medical ID offer peace of mind by ensuring EMS personnel are aware of the existence of your medical orders and know exactly where to locate them (in the case of your POLST order). The front of the metal bracelet features the internationally recognized emergency medical symbol and information about the location of your portable medical order. The back of the bracelet provides space for several lines of engraving, where you can include your name and an emergency contact phone number.
When ordering your bracelet, please select the DNR bracelet if your physician order is simply a Do Not Resuscitate form. Select the Do Not Resuscitate POLST bracelet if your physician order is a POLST with 'No CPR' checked, or select the EMS POLST bracelet if your physician order is a POLST with 'No CPR' left unchecked (indicating you want to be resuscitated following a cardiac event).
